- Golfers will choose their dinner option at registration on the day of the tournament. This year, you will have a choice of steak, chicken, ribs, salmon or tiger shrimp.
- Charitable receipts will be issued at $75 per golfer and $25 per dinner-only guest. Charitable receipts will be mailed to the mailing address you use on this form. If you would like your charitable receipt to be mailed to a different address, or split among your golfers, please indicate this in the “Additional Instructions” box, or email email@example.com.
- Payment is submitted through PayPal. Only VISA and Mastercard are accepted. Sorry, we do not take American Express. If you plan to pay by cheque, please indicate this in the “Additional Instructions” box. Cheques must be made payable to Skyline Wealth Management Inc. and can be mailed to:
