Charitable receipts will be issued by Shelldale Family Gateway at $75 per golfer and $25 per dinner-only guest. Charitable receipts will be mailed to the mailing address you use on this form. If you would like your charitable receipt to be mailed to a different address, or split among your golfers, please indicate this in the “Additional Instructions” box, or email email@example.com.
